All documents must be current and valid. If the front of a license shows that it is expired, we would also need to see the reverse side with the renewal information.

To obtain a certified copy, with raised seal, of a Death Certificate by mail, your request must include a photocopy of your driver's license or acceptable ID, the name of the deceased, date of death, your written signature, payment and return address.

*Please note that all online orders for birth, marriage, and death records must be accompanied by a signed, faxed or emailed copy of your valid Driver's License or acceptable ID.
