This form is for the March 20, 2012 General Primary.

This form can be used to request an absentee ballot for the upcoming election. This form is to be used only for registered voters in Champaign County. If you are registered to vote in another County, you will need to contact that County for your voting needs. The Illinois State Board of Elections has a listing of all election authorities in the state of Illinois, and if you go to the Illinois State Board of Elections website, you can see the entire listing.

Federal and State law now requires birth date and at least the last four digits of the social security number or the full IL Driver's License number/State ID number for voter registration. All fields must match correctly in order for the registration to be verified.

A small number of voters may not be included in this database because information such as birth date or social security was not required at the time of registration. If this applies to you, try entering your name, birth date, and a zero in the social security number field. If you still do not show up then you will need to call our office.
